0145 (5) (6)(7)(8) (9) Licensor observed that two large wooden panels had become detached from the frame structure and were found lying on the ground. As a result, the back alley area was left exposed and not properly enclosed, failing to deter unauthorized individuals from entering the premises. 0145 (11) Licensor observed personal home furnishings stacked and stored in the outdoor play space, along with wood debris leaning against the wooden fence. These items were accessible to children during outdoor play. 0145 (6) Licensor observed that the entrance gate door was missing, creating a significant safety and security concern. This condition allows children to exit the premises unsupervised and permits unauthorized individuals to gain access to the property.
